District Nurses

The district nurses attached to the practice also run a number of clinics to compliment our service such as Dressing Clinic, Good Ear Care, Continence Clinic and Injection Clinic The District Nursing team covering this GP practice runs from 08.30-17:00. Outside of these hours there is an Out Of Hours Service. If you need to change your appointment time or require an appointment with the District Nursing Team please contact them direct on 01434 323363.

Health Visitor

Our 0-19 Team (Health Visitor and School Nurses) can be contacted on Telephone 0300 373 2488.

Midwife

Our midwife, provides care and advice to pregnant mothers and their partners until the baby is six weeks old. Midwife clinics are held on Wednesday afternoons in practice and appointments can be booked through the practice on 01434 320077. If you need more urgent assistance or to speak to a member of staff within the midwifery team please contact Hexham General Hospital, Midwifery-led maternity unit on 01434 655350. Newly Pregant? Other Services

Hospital consultants also hold regular out-reach clinics in the practice and other professionals visit the practice on a regular basis, providing services including Counselling, CPN (Community Psychiatric Nurse), Dietetic Advice, Smoking Cessation, Podiatry, Diabetic Eye Screening, Drug and Alcohol Addiction Services.

PCN Care Coordinator

The Care Coordinator works closely with our practices to identify vulnerable patient groups to deliver personalised care and proactive support to; acting as a central point of contact for patients and their carers. They help patients with coordination and navigation of care and support across health and care services; connecting them to the services they require. As part of their role, they spend time with patients to establish what is important to them, developing personalised care and support plans which can be shared with other health professionals involved in their care. They support many patient groups based on the need of the patient population within our practice: Care Home: Supporting the Enhanced Health in Care Homes Service (EHCH) . Cancer: Cancer Care Reviews, Holisitic Needs Assessment, Screening uptake. Dementia, Learning Disabilities & Mental Health: Annual Reviews, Identifying Reasonable Adjustments, Signposting. Carers: Carers Health Checks, Information/Resources/Signposting. Frail/Housebound & Long Term Conditions: Annual Reviews, Signposting.

Care Coordinator

PCN Social Prescribers - Living Well Coordinators

LWCs actively link people with groups and activities, offering help to access information about benefits and support locally and enabling family, friends and carers to come up with a plan for now and the future that means lives are lived to the full way of actively connecting people to activities, groups and services that can improve their health and wellbeing. Patients may visit their GP because there are often wider challenges that people are facing beyond their medical issues. They may be feeling stressed about their work, money, or because they are lonely and isolated. But these problems cannot be fixed by medicine, or doctors alone. Social Prescribers can help patients get the support they require by offering guidance and emotional support on a range of topics for example, volunteering, arts activities, gardening, befriending, advice on debt or finances, and a range of sports and will signpost them to the relevant agencies to offer them the support required.

PCN First Contact Practioners

FCP are an experienced Musculoskeletal (MSK) Physiotherapist with professional qualifications/skills in the advanced assessment, diagnosis and management of MSK conditions. FCP services provide rapid access to specialist assessment for MSK problems. For example: - Joint Pains (New and longstanding - Muscle, tendon, or ligament problems (including sports injuries) - Spinal Pain (including neck, thoraic and lower back problems) - Gait and balance problems A patient can book an appointment with our FCP directly via speaking to a receptionist. A typical appointment involves assessment, diagnosis and first line treatment. FCP's can refer for physio treatment, order investigations or make referrals into secondary care services, if required

PCN Paramedic

Paramedics work autonomously within the community at an advanced level of practice, using their enhanced clinical assessment and treatment skills, to provide the first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or injury, abdominal pains, chest pains and headaches. They are health professionals who practice at an advanced level having the capability to make sound judgements in the absence of full information and to manage varying degrees of risk when there are complex, competing or ambiguous information or uncertainty.
